Hamill Homecare is seeking a compassionate, experienced and dedicated Complex Care Support Worker to join our growing Complex Care team.

This role involves providing high-quality, person-centred care to individuals aged 16 years and over with medically complex healthcare needs, enabling them to live safely and independently within their own homes and communities across Stirling and Clackmannanshire and to other geographical areas as the client base grows.

Our Complex Care service is clinically overseen by our Registered Complex Care Nurse, ensuring all care is delivered in accordance with best practice, robust clinical governance and individualised care plans. You will work as part of a multidisciplinary team to deliver safe, effective and compassionate care that promotes dignity, independence and choice.

This is a permanent position averaging 35 hours per week and involves a combination of day shifts and overnight shifts, including weekends and public holidays on a rota basis. Flexibility is essential to meet the needs of the individuals we support.

Key Responsibilities:

• Deliver safe, person-centred care in accordance with individual care plans and clinical guidance.
• Support individuals with personal care, nutrition, mobility, medication administration and all aspects of daily living.
• Provide care for individuals with medically complex conditions, including specialist clinical interventions, following training and competency assessment.
• Support individuals requiring interventions such as non-invasive ventilation (NIV), nasogastric (NG) feeding, PEG feeding, tracheostomy care and other complex clinical procedures within the scope of your competency.
• Monitor and accurately document changes in an individual’s physical or emotional wellbeing, escalating concerns promptly in line with company policy.
• Maintain accurate electronic care records and medication documentation.
• Communicate effectively with families, healthcare professionals and the wider multidisciplinary team.
• Support individuals to attend appointments, work, education, social activities and community outings.
• Drive individuals in their own mobility-adapted vehicles where required, ensuring their safety and comfort at all times.
• Promote independence, dignity and choice in every aspect of care.
